The Work Activity Center, Inc. employment application is an Adobe PDF (portable digital format) document. There are several ways you can complete the application.

1. You may fill the application form online and submit by email. There is a submit button at the end of the online application. After completing the application, please print a copy for your records before submitting. If you are viewing the form using Adobe reader you will only be able to print the application form. If you are using Adobe Acrobat you will be able to print and save the application form on your computer.

2. You may complete the application form online and print the completed application. Then either fax (801) 977-9791 or mail the completed application to:
